A backache that lingers past the expected recovery period is rarely just a matter of getting older or sleeping in the wrong position. When patients ask what causes chronic back pain, the answer usually involves more than one factor: a structural problem in the spine, irritated nerves, reduced movement control, an old injury, or ongoing strain that prevents tissues from fully recovering.
Chronic back pain is generally pain that persists for 12 weeks or longer. It can be dull and constant, sharp with certain movements, or accompanied by stiffness, tingling, weakness, or pain traveling into the buttock or leg. The pattern matters. Persistent pain deserves a careful evaluation because treating symptoms without identifying the source can lead to temporary relief without meaningful recovery.
What Causes Chronic Back Pain?
The spine is a coordinated system of vertebrae, discs, joints, muscles, ligaments, and nerves. A problem in one area can alter how the rest of the body moves and loads the spine. For example, a painful disc may cause protective muscle spasm, which changes posture and movement. Over time, that compensation can create additional pain in the lower back, hips, or legs.
Some patients have a single identifiable cause, such as a herniated disc after a lifting injury or car accident. Others have several contributors, including age-related degeneration, weak or poorly coordinated stabilizing muscles, and a physically demanding job. The goal of a thorough examination is not to assign a generic label to back pain. It is to determine which tissues and nerve pathways are involved, what is aggravating them, and what can be addressed conservatively.
Disc injuries and degeneration
Spinal discs act as cushions between the bones of the spine. With age, repeated bending, prolonged sitting, heavy lifting, trauma, or genetic predisposition, discs can lose hydration and resilience. This process is often called degenerative disc disease, though it does not always cause pain.
Pain may develop when a disc becomes inflamed, bulges, tears, or herniates. A herniated disc can irritate or compress a nearby nerve root, producing sciatica: burning, shooting, numb, or electric pain that travels from the low back into the buttock, thigh, calf, or foot. Not every disc finding on imaging is the source of symptoms, which is why clinical findings must be matched with the patient’s pain pattern and neurologic examination.
Facet joint irritation and spinal arthritis
Facet joints are small joints at the back of the spine that guide movement and provide stability. Like other joints, they can become irritated or arthritic over time. Facet-related pain often feels more localized than nerve pain and may worsen with standing, twisting, arching backward, or staying in one position for too long.
Arthritic changes can also narrow the spaces around spinal nerves. This narrowing, called spinal stenosis, is more common later in life and may cause pain, heaviness, numbness, or weakness in the legs during walking or prolonged standing. Some people feel better when leaning forward, sitting, or resting. The severity of imaging changes alone does not determine treatment. Function, symptoms, and neurologic findings are equally important.
Muscle dysfunction and movement compensation
Muscles can be painful, but they are not always the original problem. After an injury or disc irritation, the body often tightens muscles to protect the area. That short-term response can become a long-term pattern of guarding, reduced mobility, and poor coordination.
Weakness or delayed activation in the core, gluteal, hip, and spinal stabilizing muscles may allow repeated strain during ordinary activities. This is common in people who spend long hours at a desk, drive extensively, lift for work, or return to activity too quickly after an injury. A successful rehabilitation plan should do more than reduce pain. It should restore mobility, improve spinal control, and help the patient return to the movements that matter in daily life.
Nerve irritation and sciatica
Nerves are sensitive to compression, inflammation, traction, and reduced mobility. Irritation of a lumbar nerve root can produce symptoms far from the spine, including tingling in the toes, numbness in the foot, calf cramping, or weakness when lifting the foot. These symptoms are often dismissed as a leg problem when the source may be in the low back.
Peripheral nerve dysfunction can also coexist with spinal conditions, particularly in older adults or people with metabolic concerns. Distinguishing a nerve root problem from peripheral neuropathy, hip pathology, or vascular issues requires a focused examination. The treatment approach changes depending on the cause.
Trauma that never fully resolved
Motor vehicle accidents, falls, sports injuries, and workplace incidents can create spinal injuries that remain symptomatic long after the original event. Even a collision that seems minor at first can produce forceful loading through the neck, mid-back, low back, and pelvis. Pain may be delayed by days or masked initially by adrenaline and inflammation.
Trauma can affect discs, joints, ligaments, muscles, and nerves at the same time. When symptoms continue, an injury-focused evaluation can help identify persistent mechanical dysfunction, nerve involvement, and functional limitations. This is especially relevant when pain interferes with work, sleep, driving, walking, or other essential activities.
Why Chronic Back Pain Can Continue After Tissue Healing
Pain is not always a direct measure of tissue damage. In some cases, the original injury has improved, but altered movement patterns, ongoing inflammation, nerve sensitivity, deconditioning, poor sleep, or repeated aggravation keep the pain cycle active. That does not mean the pain is “all in your head.” It means chronic pain often requires a broader clinical view than acute pain.
Stress can increase muscle tension and reduce recovery, but it should not be used to dismiss physical symptoms. Similarly, imaging can be useful, but it is only one part of the picture. A meaningful assessment considers medical history, symptom behavior, posture, mobility, strength, reflexes, sensation, and how the condition affects real-life function.
When Back Pain Needs Prompt Medical Attention
Most back pain is not an emergency, but certain symptoms should never be ignored. Seek urgent medical evaluation for new loss of bladder or bowel control, numbness in the groin or saddle area, rapidly worsening leg weakness, fever with severe back pain, unexplained weight loss, or severe pain after significant trauma.
A timely evaluation is also appropriate when pain persists beyond several weeks, radiates below the knee, causes numbness or weakness, repeatedly interrupts sleep, or fails to improve with reasonable self-care. The earlier an underlying disc or nerve issue is identified, the more options may be available before symptoms become more limiting.
A Root-Cause Approach to Persistent Back Pain

That distinction is where individualized care can make a practical difference.
