• This fee reflects our lowest PPO insurance exam rate, extended as a self-pay introductory offer.

• This offer does not apply to automobile accident or personal injury cases, or to any visit requiring forensic, medical-legal, or attorney-directed documentation. Patients presenting with injuries related to a motor vehicle collision or other personal injury claim will be scheduled under our standard personal injury evaluation protocol, which includes the appropriate diagnostic and documentation services for their case.

Motor-vehicle-collision, personal-injury and workers'-compensation patients are not scheduled under this introductory offer — they are scheduled under our standard personal injury evaluation protocol, which includes the diagnostic testing and documentation those cases actually require. That is a more thorough visit, not a more expensive version of this one, and in most cases it is billed to the at-fault carrier, Med Pay or your attorney's lien rather than to you.
