What to have handy

Nothing here is required to submit the form — bring what you have and we'll sort out the rest together.

Photo ID and your insurance card, if you plan to use insurance
Any prior x-ray, MRI or CT imaging — or the written report
A list of medications and supplements you take
Comfortable clothing you can move and be examined in
